Care Finders Program.
A federally funded initiative by the Australian government Department of health and Aged care under the Primary Health Network programs, supporting older people who face barriers accessing aged care. Care Finders provide free, face-to-face assistance to navigate My Aged Care, link with services, and offer culturally appropriate support—especially for CALD, LGBTQIA+, and vulnerable clients. Outreach includes expos, drop-in sessions, and community engagement.
