What is the Care
Finders Program?

Care finders help older adults who may be isolated, experiencing language or literacy barriers, or without family support to access Govenment services. This program is designed for people who may not be able to connect with government services on their own.
Our experienced team will guide you every step of the way; whether you’re applying for services, attending assessments, or understanding your options. We’re here to make aged care navigation simple and stress-free.

Why Our Care Finders?

Our Care finders team are not a referral funnel for Umbrella services. Our team will work with you to link to you any and all of the services you are entitled to regardless of the provider. However, with more than 25 years of experience in multicultural services, Umbrella Community Care has become a trusted name for quality, inclusive support even when your journey doesn’t lead to our services. We’re proud to walk alongside seniors and their families to ensure they get the help they need.

How does the service work?

If someone needs help accessing health and aged care support services in Perth, a local organisation, like Umbrella, can connect them with a dedicated support person from our Care Finder team. The team member will usually meet with the person face-to-face, either in their home or at another location of their choice across the Perth metro area, including Belmont, Mandurah, and the Peel region.
During the meeting, the Care Finder team member will ask respectful, guided questions to understand the individual’s unique circumstances. This personalised support is especially valuable for older people from CaLD backgrounds who may benefit from culturally appropriate aged care in Perth.
The Care Finder team is also available to run outreach sessions for specific language, cultural or community groups. If you are interested hosting the Care Finder team, please get in touch by emailing cf@umbrellacommunitycare.com.au or through the contact portal below.

What help can care finders provide?

Care finders assist older people in navigating the aged care system and accessing the right supports. They can help with:
  • Explaining and helping access home support services in Perth
  • Talking to My Aged Care on the person’s behalf and arranging assessments
  • Attending assessments and offering emotional support
  • Finding and shortlisting aged care providers in Perth, and surrounding areas
  • Completing paperwork and explaining aged care service agreements
  • Checking in after services commence to make sure everything is running smoothly
  • Connecting people to other community services, including:
    • Health and mental health services
    • Housing and homelessness support
    • Drug and alcohol services
    • Senior community programs and senior entertainment in Perth
